Your personal data will be secure and confidential - this is of primary importance to all of us at Club CB. We must have this information so that we know the real identities of our members, for everyone's protection, though no-one else will know. You will receive an email requesting you to schedule a telephone conversation with both of you to discuss your application. The application process includes acceptance of our Members Disclaimer agreement, which you can read below.  We'll call you on a convenient evening to explain how the Club works, and help you prepare for this experience. Please fill out the requested information completely, including both partners first names in the appropriate section, in the form Mary & John. These are the only names we will use at the Club and you are welcome to use nicknames.

1. We are both adults, at least 21-years of age.

2. We certify that all the details of our application are correct and that we are not misrepresenting ourselves.

3. We warrant that, if either of us is a police officer, government investigator or agent of any kind, we will be registering as Members, using the website, and attending any Club event only in our private, individual capacity. We acknowledge that our presence at any Club event without disclosing our official capacity and investigation purpose, would be an entry into private premises without consent, in violation of the 4th Amendment to the U.S. Constitution.

4. We are not religious persons intent upon harassment or upon imposing our religious ideas or beliefs onto anyone.

5. We are not from any media whose intent is to malign, harass, embarrass or destroy any person's name, character or livelihood. We represent that, if either of us is a member of the press (including television & radio), we will be registering as Members, using the website, and attending any Club event only in our private, individual capacity. We acknowledge that, if we attend at the direction of our employers and are acting within the scope of our employment; that we are violating the rights of the Members to privacy under common law and the U.S. Constitution, and that an appropriate amount of liquidated damages for any published report made which portrays or describes the activities of any Member is payable to compensate for loss of privacy, humiliation, emotional distress, necessity to pay legal fees, loss of employment or advancement, and other injuries. Such damages apply to each Member named in the report or described in such a way as to permit identification of the Member by application of the description to publicly available information.

6. Both of us wish to become members and attend Club events of our own free will.

7. We agree with and intend to abide by the policies of the Club as to "any reason for an apology is reason for not being here", that "No means No", and "to be polite and considerate of other’s privacy".

8. We save harmless the establishment and the club for any and all damages that might occur to me/us due to an unforeseen accident.

9. We understand that the use of any illegal substance, immediately before attending or during Club events will result in eviction from the premises and permanent cancellation of our membership.

10. We are and will not be offended by nudity, sexual acts, or lewd behavior.

11. As members of this private Club, we intend to honor and protect the privacy and well being of the Club and all our fellow members.

We agree that by requesting a telephone interview, we accept the terms of this disclaimer and fully support the stated policies of the Club.
